Introduction
Thrive works with Local Authorities, schools, social care, families and other professionals to provide personalised alternative education.
We understand that commissioning an AP placement requires more than finding a space. The placement needs to be suitable, safe, purposeful and capable of demonstrating progress.
Our referral process
- Referral received
2. Referral and EHCP information reviewed
3. Suitability and risks considered
4. Family and learner engagement
5. Site visit or transition activity where appropriate
6. Timetable and placement agreed
7. Baseline assessment
8. Personalised learning begins
9. Progress monitored and reported
10. Review and transition planning
Information we need
To assess suitability, we may request the learner’s EHCP, referral information, attendance history, relevant risk information, safeguarding information, medical information, current attainment, behaviour/regulation information and the learner’s and family’s views.
We use this information to determine whether Thrive can safely and appropriately meet the learner’s needs.
For commissioners
We aim to provide clear communication, transparent progress information and a planned approach to transition. Where the placement is intended to be temporary, transition planning begins from the outset rather than at the point of notice.
